Schools pretty much decide their own policy regarding testing and sanctions. For example, Stanford doesn't test any athletes at any time (McCafferty anyone?). I believe only at bowl game time could one get popped and if you decide to forgo the bowl and get ready for the combine .... well then no.
There is a really good article in the WSJ about it, but it is behind a paywall. But they do have a nice table showing each schools policy that you can see without paying.

Below is a link from 2016 where Saban complains that the only teams tested that year are the 4 playoff teams and he views that as a competitive disadvantage.
